The 30-day follow-up

The same seven areas, the same 1–5 scale, thirty days on. This is not a satisfaction survey and there is no right direction for the numbers to move.

This is a reflection tool, not an assessment of educator effectiveness. It is not a grade, not an evaluation, and it is not shared with your school or district. Do not enter any student information.
